Cleanup handling of UniqueExternalLinkage.
This patch renames getLinkage to getLinkageInternal. Only code that needs to handle UniqueExternalLinkage specially should call this. Linkage, as defined in the c++ standard, is provided by getFormalLinkage. It maps UniqueExternalLinkage to ExternalLinkage.
