code headers accordingly. In the same go we should also fix the links to
point to qt.io.

- Replace Nokia contact email address with Qt Project website.
- Remove "All rights reserved" line from license headers.
As in the past, to avoid rewriting various autotests that contain
line-number information, an extra blank line has been inserted at the
end of the license text to ensure that this commit does not change the
total number of lines in the license header.

QPixmapCache has until now refused to throw out shared pixmaps, i.e.
ones that still have shallow copies lying around. This leads to problems
when someone inserts two shallow copies using different keys, causing
the cache itself containing multiple shallow copies and thus forever
refusing to throw out those entries.
It's rather easy for this to accidentally happen in a user application
since QPixmap::load() or QPixmap(const QString &fileName, ...)
automatically cache the pixmap in the QPixmapCache, thus if the user
then calls QPixmapCache::insert() on the same pixmap or a shallow copy
it is locked in the QPixmapCache forever.
The only reason for not throwing out a pixmap that's shared would be to
prevent re-loading a pixmap from file when a user has a direct reference
to it in his application, but in that case the user is unlikely to
re-load the pixmap from file in any case. Therefore it seems the best
fix is to get rid of this limitation.

Removing a pixmap from the pixmap cache using the new QPixmapCache::Key
API left the keys dangling in the QCache's internal QHash. The problem
is that the Key is invalidated as soon as the QPixmapCacheEntry is
destroyed, thus removing it from the hash failed. Reordering the
destruction of the object and the removal of the key in QHash fixes the
problem.

The default cache_limit in different platforms have different value.
For example in Symbian the default is currently 1024 KB where as for
desktop platforms the default is 10 MB.
The purpose of modified qpixmapcache test cases was to do operations
until cache_limit was reached. This was achieved by hard coded 40000
iterations. However this hard-coded value is fargile for cache limit
changes, and in addition it unnecessarily made the test exectuion to
take very long time on platforms which had smaller cache limit.
This patch changes the test so that number of expected items to fit
in cache is calculated and then 1000 extra items is tried to put
in cache to make sure limit is exceeded.

There was a bug in QPixmapCache when QCache trims the content, some keys
were not invalidated. The ifdef for WinCE (that i removed) was a wrong
fix, it let the auto-test pass but it doesn't fix the bug. The approach
here is to add a QPixmapCacheEntry that release the key it owns when
QCache deletes it : we are now sure that nothing happen in our back.

This commit add a new API to add/find/remove pixmaps into QPixmapCache.
This new extension is based on a key that the cache give you during
the insertion. This key is internally a int which makes all operations
in the cache much more faster that the string approach.
Auto-tests has been extended as well and a benchmark has been added to
compare both approach. I also depecrate the find method for the
string API to have a method pointer based and not reference based like
the Qt policy says.

If you insert a new pixmap in the cache with a key that was already in
the cache then we remove the old pixmap and add the new one. This avoid
to fill the memory with garbage even if the cache has a protection to
avoid running out of memory. This was discovered with QraphicsView
and its cache. We don't need to keep old cached pixmaps for an item.
