Cocoa: Add initial accessibility implementation.

See qcocoaaccessiblity.h for details.

For now only the first level of the hierarchy is
made accessible.

Also add tools/accessibilityinspector which is an
utility for inspecting and debugging the Qt
accessibility tree.
