Replace the current license disclaimer in files by
a SPDX-License-Identifier.
Files that have to be modified by hand are modified.
License files are organized under LICENSES directory.

Use new global include snippets for the 'Using the Module' section and
reorganize the sections structure. The sections should follow this
order:
- Using the Module
- Articles and Guides
- Examples
- (API) Reference
- Module Evolution
- Licenses and Attributions

So far the pages were only 'rooted' in the separate qtcmake
documentation. Let them have a page per module too, so that
it's easy to navigate.
At the same time rename the cmake-macros-qtcore group to
cmake-commands-qtcore. This will require a fixup in qtdoc
repository.

The content in the porting guides are closer to a changelog than
a porting guide. At this point, it is easier for maintainers and
contributors to write in a changelog than a guide. This change
should help with readability and is closer to the usage of
"Changes" in documentation.
Part of a rename in other submodules.

The use-case is relevant for other platforms as well.
Now that Qt has a module system we can also replace a lot of the
hand crafted logic for linking with simpler constructs.

Add documentation on how to use a module from CMake, alongside
the existing documentation about qmake. Separate generic info
from module-specific examples, to make it possible to use one
include file in all modules.
While at it, also remove the mentioning of the central include;
it is not something we should actively advocate anymore. Instead,
the documentation of every class gives the correct include to use.

We do add, remove or update third party code in minor releases,
sometimes even in patch level releases. However, the documentation is
supposed to be valid for all existing Qt 5 versions, but doing this for
attributions would require infrastructure we don't have.
Therefore rather make it explicit which Qt version the attributions
apply to. Also mention since when Qt is available under LGPLv3
(starting with Qt 5.4).

The documentation for the qtmain license is right now pretty hidden.
The library is added by qmake independent of the Qt module that are
linked to, but it seems safe to assume people at least link against
Qt Core.

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.
Outdated header.LGPL removed (use header.LGPL21 instead)
Old header.LGPL3 renamed to header.LGPL3-COMM to match actual licensing
combination. New header.LGPL-COMM taken in the use file which were
using old header.LGPL3 (src/plugins/platforms/android/extract.cpp)
Added new header.LGPL3 containing Commercial + LGPLv3 + GPLv2 license
combination

The majority format is "<Qt Module> C++ Classes" (see
http://qt-project.org/doc/qt-5.0/qtdoc/modules-cpp.html)
Also, fix a broken link (Qt Network C++ Classes)
"<Qt Module> C++ API" is perhaps the more correct format, but that's
part of a much bigger cleanup: QTBUG-30556
