Note that this is a mere search/replace; some places were using double braces to work around the presence of commas in a macro, no attempt has been done to fix those. tst_qglobal had just some minor changes to keep testing the macro. They are kept in order for a linear layout, so that users see them in the right order, but there's no real guarantee for that anyway if combined with spacers and other non-items. Continue to try keeping the order, but ensure indices that are out of bounds are treated as appends to the list.
