This reverts commit dfa8854cf7e00705e0122cf7022ff1ea4f8e5a74 and
removes all "selected" signal emissions from the Cocoa platform
dialogs.
Even though it fixed the new QML dialogs that were relying on the
"selected" signals, it lead to duplicate signals with QColorDialog,
QFileDialog, and QFontDialog. We'll fix the new QML dialogs to not
rely on the selected signals, but handle it on accept the same way
than QtWidgets and QtQuick Dialogs do, so there is no need to repeat
the signals in all platform plugins.

When we set accessory view sometimes (sic!) a delegate's callback
fires: -panel:directoryDidChange: with an outdated path (probably because
panels are shared?) resetting our current directory; later we open
file dialog with a wrong path as result.

because in QtQuick.Controls, we do not call exec(): we just set the
dialog visible. If it is a sheet, then it is already acting as a
modal dialog, basically.

Previously, for Qt::WindowModal dialogs we were creating modal sheet with beginSheetModalForWindow:
and later (from exec) calling -runModal, essentially making a window modal dialog into (now) application
modal sheet, which is not right- it's centered relative to desktop and the jump from window modal sheet's
position (centered relative to a window) to application modal was quite visible.
Now, instead of [panel runModal] (== [NSApp runModalForWindow:panel])
we call [NSApp runModalForWindow:theRequiredWindow].

We have at least 5 different (but equal) implementations of a wrapper
in Qt, and some code uses explicit NSAutoreleasePools. Having a shared
implementation lets us clean up things a bit and makes it easier to
reason about which pools are actually needed.

Whenever the selection changes it will call panelSelectionDidChange
even if the panel is not visible which causes it to overwrite our
chosen initial file with "untitled". Therefore we only set the name
when the panel is actually visible.

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.
Outdated header.LGPL removed (use header.LGPL21 instead)
Old header.LGPL3 renamed to header.LGPL3-COMM to match actual licensing
combination. New header.LGPL-COMM taken in the use file which were
using old header.LGPL3 (src/plugins/platforms/android/extract.cpp)
Added new header.LGPL3 containing Commercial + LGPLv3 + GPLv2 license
combination

This bug can be reproduced (AFAIK) only on 10.9. To fix it I suggest we create
NSOpenPanel/NSSavePanel
_every_ time it must be displayed. Actually, that's what I've seen in all code samples
I was able to found - nobody tries to retain this panel and re-use it.
If we re-use it, "Media" section magically disappears.
I believe this bug is not Qt's bug, but something weird in Cocoa.

In some language, mnemonics put after label text within parentheses.
e.g. "&Open" is translated to "開く(&O)" in Japanese.
OS X doesn't use mnemonics and '&' in label text is removed.
Mnemonics in parentheses (and spaces before them) also should be removed.

Now menu items and key shortcuts for Cut, Copy, Paste and Select All
work in the standard ways in dialogs such as the file dialog, provided
that the corresponding QActions have been created and added to the menu.
This depends on new roles to identify each menu item which is so
broadly applicable that it should work even when a native widget has
focus; but the role will be auto-detected, just as we were already
doing for application menu items such as Quit, About and Preferences.
When the QFileDialog is opened, it will call
redirectKnownMenuItemsToFirstResponder() which will make only those
"special" menu items have the standard actions and nil targets. When
the dialog is dismissed, those NSMenuItems must be reverted by calling
resetKnownMenuItemsToQt(), because to invoke a QAction, the NSMenuItem's
action should be itemFired and the target should be the
QCocoaMenuDelegate.

QFileSystemWatcher causes sandboxing errors since its backend uses
POSIX API in a relatively liberal way.
Also, Cocoa already acts as a file system watcher, and calls
-[QNSOpenSavePanelDelegate panel:shouldShowFilename:] on each
file. From a logical point of view, caching the directory content
can be replaced by testing the current file against the filter
setting. We expect Cocoa to cache results, and by using NSFileManager
things should remain relatively fast.

Currently, in save mode, if selectFile has not been called, the native
cocoa file dialog puts the folder name in the line edit. This patch
restores the old behavior where the line edit is presented empty to
the user.

Objective-C++ does not support namespaces. In order to make this work,
we decorate each symbol with suffix using the macro QT_MANGLE_NAMESPACE.
However, with such a technic each public symbol needs to be explicitly exported through
this macro. The use of @compatibility_alias allows to define alias to use it
automatically and transparently.

Since we have to add the filters one by one to the Mac file dialog it
was finding the one that would match the filter by comparing the start
of the filter string. However it would continue to check the start of
other filters even if it had already found the one it should be using.
Now it uses either an exact match or the first one that it matches the
start of.
