Set Qt::AA_DisableShaderDiskCache when the feature "disable_program_cache"

On VMWare Workstation 12 it will indicate OpenGL 2.1 support but it is
not sufficient enough as it is lacking things needed to use QtWebEngine
without crashing. Falling back to d3d9 works fine in this case as d3d11
also crashes.

The combination of vendor and device ID being 0000 indicates, that the
standard VGA driver is used, and happens when using Qt in a:
- Windows 7 machine without proper GPU drivers
- Windows 7 machine with disabled GPU
- HyperV vm
The default driver does neither support D3D9 nor D3D11 properly so that
we have to fall back to warp mode for ANGLE.

the windows driver for Intel HD Graphics 3000 is buggy. the original
blacklist was using device ids from:
http://www.pcidatabase.com/search.php?device_search_str=graphics
however this list seems to be incomplete. to be on the safe side, we should
blacklist all 6th generation devices, as taken from:
https://en.wikipedia.org/wiki/List_of_Intel_graphics_processing_units#Sixth_generation

By adding support for the driver description, we can detect if
we are in VMware. In this case D3D9 and 11 get disabled, so only
the software-based options are in use.
This allows running autotests like tst_qopengl, tst_qopenglwidget,
tst_qgl, etc. in the Qt CI system. There OpenGL 2.x is not available,
so ANGLE is the only option. D3D11 is not an option, so it picks D3D9
by default. However, this results in mystic failures. The stable solution
seems to be to use WARP. This can be achieved by setting disable_d3d9 in
the built-in GPU blacklist.

The original rule was way too broad: it disabled desktop GL many
HD 4400 machines that have no problems with it at all.
While the rule with the version check is somewhat dubious, it is the
only thing we can do.

Use a built-in JSON file in case the QT_OPENGL_BUGLIST environment
variable is not set. When QT_OPENGL_BUGLIST is set, the built-in list
is ignored.
To make the implementation simpler and more readable, some of the code
in QWindowsOpenGLTester is reshuffled a bit. It also caches the results
now, so it is safe and fast to call supportedRenderers() and friends
multiple times.
The blacklist currently contains the Intel card from QTBUG-43263 (Intel
GMA / HD3000 ?) and may also apply to QTBUG-42240.
[ChangeLog][QtGui] Qt now contains a built-in GPU driver blacklist for
Windows that disables the usage of desktop OpenGL with some older cards
that are known to be unstable with opengl32.dll.
