Instead of generating .html page for each file in an example
project, generate links to code.qt.io, under the correct
path and branch, where the user can browse the example source.
Store all URLs under QT_INSTALL_DOCS/config where other qt5
submodules can access them. The repository name appears in
the URL, so we cannot define a single URL for all modules.

Qt Test library sources specific to Core, GUI and Widgets modules
were moved around in commit 88867e39b.
The new source locations must be referenced in Qt Test documentation
configuration. The same sources are excluded in their original doc
projects, and the related snippet file is moved over to qttestlib.
The commit also fixes the remaining documentation issues for Qt Test.

The update adds the moduleheader variable to the qdocconf
file for qttestlib.qdocconf and qmake.qdocconf. The problem with
qmake is that it doesn't have a module header file, but it does
have qmake_pch.h, which is used here. This update also corrects
several \fn commands in the qttestlib docs.

The example directory name for Qt Test module is qtestlib.
Without this change, Qt Creator cannot find the tutorial
files and tags them "broken" in the Welcome mode.

The examplesinstallpath variable in .qdocconf files defines the path
under QT_INSTALL_EXAMPLES where examples are found.
To match the way examples are packaged in Qt 5.6, prefix each
install path with the repository name.

In 5.2, the HTML output is in a flatter structure and when they are
hosted in qt-project.org/doc, the documentation will be found at
http://qt-project.org/doc/qt-$QT_VER
The url variable is used by projects outside of Qt 5 which need
to link to Qt 5 documentation, such as Qt Creator.

QtTestLib and QTestLib don't exist. The proper name is "QtTest" (code)
or "Qt Test" (English)
http://qt-project.org/wiki/Spelling_Module_Names_in_Qt_Documentation
http://lists.qt-project.org/pipermail/interest/2012-December/005221.html
Files paths in qttestlib.qdocconf can't be changed easily however, as it
breaks things. So, they're left as they are.
