When storing the persistent indexes within _q_sectionsAboutToBeChanged()
there was a limit one how much persistent indexes should be stored due
to performance concerns. This resulted in unhiding hidden rows.
The rationale behind this was that storing persistent indexes is slow
and that hiding a large amount of rows/columns should be done with a
QSortFilterProxyModel.
Fix it by removing the limit since it does not really hurt performance.

If there is a parent (typically an itemview) then StatusTip events
should be sent to that. However in the case of there not being a parent
then the event should be sent to the QHeaderView itself.

The code that updates a section size must also update length,
otherwise saveState can end up saving inconsistent state, and
restoreState() goes to an early-return, not doing anything.
The actual bug was fixed meanwhile because _q_sectionsChanged is called
again, which recalculates length. I still see this only as a safety
measure, every other code path that changes section sizes updates length
right away.

Do not clear the settings of QHeaderView during layoutChange when the
model is empty and the section count did not change. This will not work
when a section is moved or a section is replaced with a new one during
layoutChange. But since layoutChanged is also called on sorting, this
patch ensures that the settings are not cleared in this case.
This restores the behavior to the same as before 5.9.4.

QHeaderView::resizeSection() did not check if the given section size is
inside the min/max property bounds.
Also on calling setMin/MaximumSectionSize() the current section sizes

QTreeView makes columns movable except for the first one
(since Qt 5.0, see commit e0fc088c0c). This is indeed best
for actual trees, but not when using QTreeView as a flat
multi-column list (with no "root decoration"). Then it should
be possible to move the first column.
[ChangeLog][QtWidgets][QHeaderView] Flat treeviews can now allow
the user to move the first column (like in Qt 4.x) using the new
method QHeaderView::setFirstSectionMovable().

QHeaderViewPrivate reimplemented _q_layoutChanged() to handle changes
of rows/columns via layoutChanged/layoutAboutToBeChanged. This worked
fine for Qt4 but since Qt5 only the special signals rowsAboutToBeMoved/
rowsMoved are used for this (8021e2d5e7ccd09146896f788441c116f2ca6159).
With this change, QAbstractItemViewPrivate::_q_rows/columnsMoved() is
calling the virtual function _q_layoutChanged(). This resulted in a
wrong call of QHP::_q_layoutChanged() for a horizontal header when
a row changed and for a vertical header during a column change. In the
end this can lead to an unhide of hidden sections.

A layoutChange indicates that anything can have moved to anywhere else,
including as a result purely of new items being added. It can also
indicate that items are removed.
The old code here incorrectly assumed that the section count remained
constant over this operation by setting the size of the oldSectionHidden
QBitArray - whose size is the size before the layoutChange operation -
and then calling setBit with model rows numbered after the layoutChange
operation. As the two are not necessarily the same dimensions, this can
result in asserts from the setBit call.
Simplify the handling of layoutChanged entirely by clearing section
information, and using the QPersistentIndexes which indicate hidden
state to restore that state after re-population.

This is a followup to 77a8e90cddcfa1c34518ef846a4838874a7bc0c7 which
didn't handle the case where no columns had been moved.
visualIndices and logicalIndices are empty until initializeIndexMapping()
is called, in which case appending is wrong.
As a result, visualIndex(i) would return -1 for the values over
those added by read(), and an assert would happen at painting time.
The fix is to leave visualIndices and logicalIndices empty if
they are empty already, leaving it to initializeIndexMapping()
to fill them later if necessary (e.g. when moving a column).

state onto a 5-column headerview, the headerview shouldn't suddenly think
it has 3 columns.
Rather than making restoreState() fail, we adjust for the additional
columns, so that we can still apply the customizations from the user to
all other columns (hiding, moving, etc.).

Commit c5db8fc74 changed all instances of Q_WS_FOO to have the prefix
Q_DEAD_CODE_FROM_QT4 instead, to make it clearer when reading the code
that the code in question was a left-over from Qt4, when we used
Q_WS_ defines instead of Q_OS_ defines.
This worked well for cases of #ifdef Q_DEAD_CODE_FROM_QT4, but less so
for cases of #ifndef Q_DEAD_CODE_FROM_QT4, where the code was actually
unconditionally included.
To make this even clearer, the defines have been replaced by checks for
1 or 0, with a comment describing how the code used to look in Qt4. The
use of constants in the check also makes it easier for editors to parse
the condition and show visually that the code is defined out.

Replaces our mix of comments for annotating intended absence of break
in switches with the C++17 attribute [[fallthrough]], or its earlier
a clang extension counterpart.

The old code incorrectly calculated visual region for a selection (specified by
QItemSelection object) in case of reordered / swapped rows or columns. It used
the leftmost and rightmost (upmost and downmost for vertical mode) logical
indexes directly. However some middle logical index (in case of reorder) may be
the leftmost or rightmost visual index. In such cases the repainting didn't
work properly. This fix first checks whether reordering / swapping is in use.
If it isn't (ie visual=logical) we use code similar to the old code. Otherwise
the new code scans all selected logical indexes, translates them into visual
ones and gets the correct leftmost and rightmost indexes.
[ChangeLog][QtWidgets][QHeaderView] Fixed a repainting issue when items had been reordered.

For some reason this setter wasn't protected against setting
the same value again, and always did work (including resizing
the last section when setting it to false).
