By rendering the focus ring directly on the backing NSView, we
would ignore the painter's clipping information. It would also
require creating a custom CGContext and attached NSGraphicsContext
every time.
The first step is to render the focus ring on a pixmap and then
use the painter to render that pixamp. This ensures the clipping
is done properly. The second step is to cache said pixmap and
render it as a nine-patch image.

This change partially reverts 1bfc7f68 about QT_HAS_BUILTIN define
and undef in src/corelib/tools/qsimd_p.h.
This change is also squashed with "Fall back to c++11 standard
compiler flag for host builds" which is done by Peter Seiderer.

On OS X, the default scrolling mode of item views should be
per pixel instead of per item. We enforce this through a new
style hint. On all other platforms, the behavior remains the
same.
It's still possible to override the style hint by using the
regular scroll mode setters. Any subsequent style change will
result in a no-op once the setters have been called and until
the properties are reset.
Some auto-tests had to be update to to take the new behavior
into account.
[ChangeLog][QtWidgets][Styles] Added SH_ItemView_ScrollMode
style hint.
[ChangeLog][QtWidgets][Item Views] Item views scroll per pixel
on OS X now.
[ChangeLog][QtWidgets][Item Views] QAbstractItemView::verticalScrollMode
and QAbstractItemView::horizontalScrollMode are now resettable.

Repeatedly calling QVector::erase(it) (via QMutableVectorIterator::remove())
results in quadratic runtime.
Separate the removal of expired objects from the sending of the event to
the remaining objects, so we can use QVector::removeAll() for the former,
which does exactly what the old code tried to do, except in linear time.
Use range-for for the sending-loop. This could cause detaches, but since
we modify the container two lines before, and we don't copy it anymore,
anywhere, detaches are impossible.

The printf-style version of QDebug expands to a lot less code than the
std::ostream-style version. Of course, you pay in type safety (but
compilers warn about it these days), you cannot stream complex Qt
types and streaming QStrings is awkward, but in many cases you
actually improve on readability.
But the main reason is that something that's not supposed to be
executed under normal operation has no business bloating executable
code size.
This is not an attempt at converting all qWarnings() to printf-style,
only the low-hanging fruit.
In this first part, replace
qWarning() << "...";
with
qWarning("...");

Cocoa is basically just AppKit + CoreData. Since we do not use CoreData
in Qt, there is no reason to link to it or (transitively) import its
headers.
This is just a mechanical replacement of -framework Cocoa with
-framework AppKit and <Cocoa/Cocoa.h> with <AppKit/AppKit.h>

Work around QPaintDevice::metric's int return type
by adding a new metric that returns a scaled devicePixelRatio.
Choose a scale factor that gives us more than enough
range.
The QPaintDevice::devicePixelRatio() convenience accessor
is public API and can unfortunately not be changed
to return a qreal. Add devicePixelRatioF() which
returns the (unscaled) devicePixelRatio.
Change all call sites of QPaintDevice::devicePixelRatio()
to use QPainDevice::devicePixelRatioF().

Holding a mutable item in a QSet violates QSet invariants, namely that in
bucket N, all items have hash % size == N. If a value gets reset to
nullptr by external means, that invariant no longer holds. The code works
by pure luck.
Replace the set with a vector, which doesn't care whether elements change
under the hood, and will also outperform the typical QSet use due to
better locality of reference.

Note that at one point we need to use Cocoa to render the combo box, but
only if we're dealing with Qt Quick controls.
Also worth noticing, there's currently a bug in Cocoa when rendering
inactive combob boxes. We faithfully reproduce it in Qt for now. We'll
fix it when Apple does.
Finally, we need to start constraininig the combo boxes height. Cocoa
has not supported variable height combo boxes for years, and will even
spit the following warning if we try to do something smart.
This application is trying to draw a very large combo box, 28 points
tall. Vertically resizable combo boxes are not supported, but it
happens that 10.4 and previous drew something that looked kind of sort
of okay. The art in 10.5 does not break up in a way that supports that
drawing. This application should be revised to stop using large combo
boxes. This warning will appear once per app launch.

Some offsets differ between QWidgets and Controls. Therefore, we need
drawNSViewInRect() to know about the origin of the call.
