Fix the condition introduced by that determines whether
a completion is started on signal QFileSystemModel::directoryLoaded()
(introduced by 416ec00e7c859a844a5bcb24c7a31147aed974c / Qt 4).
Observe case sensitivity and the native separator and return true for

Due to an implementation detail regarding setParent() and
setWindowFlags(), if a QCompleter popup ends up being the last open
window after closing the main window, the application would be
prevented from exiting even after selecting an item in the popup.
This is due to adjustQuitOnCloseAttribute() not being called when
passing a Qt::Popup flag to setParent(parent, windowFlags).
Use setWindowFlags() directly, which adjusts the quit on close
attribute, and allows an application to exit properly.

This prevents unnecessarily removing and reinstalling the completer
as event filter on the same widget.
This does not prevent what's going on in QComboBox::focusInEvent(),
where we'd set the line edit as widget just to immediately override
it with combo box itself.

In matchHint() and lookupCache(), instead of checking for the
existence of 'parent' in 'cache', a new entry was created, if
needed.
However, the remainders of the functions could only fail in
that case, so don't allocate an empty map node here at all.
Wherever something sensible is added to the cache, there will
have to be an insertion of the same node anyway, so there's
no point doing that work here.
Allows to mark these functions const.
Also passed the QString argument by const-reference instead
of by value.

The algorithm used was:
- If possible, just declare the container const
- Otherwise, for first()/last(), use constFirst()/constLast()
and for front()/back(), to not destroy the use of the STL
API subset, use qAsConst()
Did some caching of function returns here and there, and
converted one 0 to nullptr as a drive-by.
Also saves almost 4KiB in text size on optimized GCC 4.9
Linux AMD64 builds.

If, after checking a condition, we issue a qWarning(),
by definition that check is unlikely to be true.
Tell the compiler so it can move the error handling
code out of the normal code path to increase the
effective icache size.
This change contains the changes to the util/,
dialogs/ and widgets/ subdirs.
Moved conditional code around where possible so that
we could always use Q_UNLIKELY, instead of having to
revert to Q_LIKELY here and there.
In QSystemTrayIcon::setVisible(), as a drive-by, I
swapped the evaluation order of an &&-expression
(newly wrapped in Q_UNLIKELY) to be more readable
and more efficient (cheaper check first) at the same
time.

The logic for handling cancel operations was spread out through
the code base and sometimes hard-coded to only include the Escape
key shortcut, missing the Command+. shortcut.
We now intercept both attempts at cancel operations from the system
through cancelOperation, which we forward as normal key events.
A new QKeySequence::StandardKey has been added for the Cancel sequence,
which maps to Escape on all platforms, and Command+. in addition for
OS X. The hard-coded logic in QWidget and subclasses for dealing
with closing the dialogs has been replaced with this key sequence,
which allows clients to override the behavior. Note that the widget
code is not wrapped in checks for QT_NO_SHORTCUT, as we don't care
about keeping widgets building and working under that define.
The logic in QCocoaWindow to bypass windowShouldClose when delivering
IM events has been removed as we now handle that specific case by
also forwarding Escape as a cancel operation.

code headers accordingly. In the same go we should also fix the links to
point to qt.io.

QCompleter::setFilterMode(Qt::MatchContains) will enable filtering
out entries that contain typed characters in any place, instead of the
default behavior when only those entries that start with typed characters
are displayed. Qt::MatchEndsWith is also possible.
QCompleter::setFilterMode(Qt::MatchStartsWith) will bring the default
behavior back.

'TAB character in non-leading whitespace' issues have been fixed
to make the sanity bot happy. Plus some general ws-fixes here
and there as asked for during review.
