With the current API it is not possible to reset the
index into -1. We have setClean() method, but
we are lacking setDirty(). This is needed
in case when the document has changed outside
of the editor and nothing has changed
in the undo stack history. In this case we
don't know the state of the file modified
externally so we need to mark that editor's
contents is different from the file contents
and undoing or redoing commands can't bring
the editor to the clean state.
This may also be useful to call it when
we created a new document and haven't saved
it yet or when the document was restored
from backup file.

Commit c5db8fc74 changed all instances of Q_WS_FOO to have the prefix
Q_DEAD_CODE_FROM_QT4 instead, to make it clearer when reading the code
that the code in question was a left-over from Qt4, when we used
Q_WS_ defines instead of Q_OS_ defines.
This worked well for cases of #ifdef Q_DEAD_CODE_FROM_QT4, but less so
for cases of #ifndef Q_DEAD_CODE_FROM_QT4, where the code was actually
unconditionally included.
To make this even clearer, the defines have been replaced by checks for
1 or 0, with a comment describing how the code used to look in Qt4. The
use of constants in the check also makes it easier for editors to parse
the condition and show visually that the code is defined out.

As of version 10.12 (Sierra), the name of Apple's desktop operating
system will be macOS. Change the occurrences where the Mac platform
is discussed to use a macro \macos, which expands to 'macOS'. This
helps with adapting to future renaming.
Update the instructions on mac-specific Q_OS_* macro usage.
Add a \target for the old 'Qt for OS X' topic to keep links working
for other documentation modules that try to link with the old name.

The new modular configuration system requires one global
header per module, that is included by all other files in
this module.
That header will later on #include the configuration file
for Qt Widgets. For now it defines the Q_WIDGETS_EXPORT
macro for this library.

The QScrollerTimer instance allocated by QScrollerPrivate was never
freed. I've absolutely no idea why QScroller has such a messy
d-pointer handling, so go for the easiest approach: exploit the
fact that QScrollerPrivate is a QObject and parent the QScrollerTimer
to it.

This is the simplest of the patch series: Q_FOREACH took a
copy, so we do, too. Except we don't, since we're just
catching the return value that comes out of the function
(RVO). We can't feed the rvalues into range-for, because
they are non-const and would thus detach.
Saves 2.2KiB in test size on optimized GCC 5.3 Linux AMD64
builds.

The algorithm used was:
- If possible, just declare the container const
- Otherwise, for first()/last(), use constFirst()/constLast()
and for front()/back(), to not destroy the use of the STL
API subset, use qAsConst()
Did some caching of function returns here and there, and
converted one 0 to nullptr as a drive-by.
Also saves almost 4KiB in text size on optimized GCC 4.9
Linux AMD64 builds.

It's a virtual function call, followed by another out-of-line
function, so the compiler cannot hoist the expression out of
the loop itself.
But we can, so do it.

There's no point in checking isEmpty() to prevent a for loop from
executing, esp. if the checking involves copying the container
and calling an out-of-line function to get said copy.
Just fall through the for loop.

For qDebug() and qWarning(), this is just an optimization.
For qCritical(), which can be fatal, the old code was just wrong.

This information is already registered by the QMessageLogger ctor.
Where, by dropping the << Q_FUNC_INFO in ostream-style qDebug(), only
a string literal remained, converted to printf-style qDebug() on the
go.
