A QDockWidgetItem will be leaked if a QDockWidget is dragged out of a
floating tab window, and then plugged back somewhere.
The problem is that QMainWindowLayout::unplug was not returning the
QDockWidgetItem* from the floating tab's layout. When that's the case,
a new QDockWidgetItem is created in QDockWidgetPrivate::startDrag
and will be put into the layout, leaking the old QDockWidgetItem.

The problem was caused by commit 0feeb6f6d2cfaa964763ca1fcab65672812b4eef
which fixed QTBUG-58036. It reparented widget that used to be skiped.
In particular, floating widgets are skiped. But seting the parent of a
floating widget docks it. And so as a result it would not be skiped anymore.
This has two side effect: This breaks the animation (as the widget is
docked too early; and cause crash when QDockWidgetGroupWindow get
reparented as this breaks invariant that these are always floating.
So restore the skip from before commit 0feeb6f6d2cfaa964763ca1fcab65672812b4eef,
and explicitly set the parent in all cases when the animation finishes.

The rubberband is shown depending if there's a current hovered dock widget,
but there were a few places that were not calling updateGapIndicator().
Additionally, the rubberband will also disappear if the currentHoveredFloat
is destroyed externally (would leave a ghost rubber band behind).

Qt doesn't create the actual QDockWidgets when restoring, the user must
ensure they are created before restoring state. So lets not create an empty
QDockWidgetGroupWindow which you can't close and with no tabs.

Commit c5db8fc74 changed all instances of Q_WS_FOO to have the prefix
Q_DEAD_CODE_FROM_QT4 instead, to make it clearer when reading the code
that the code in question was a left-over from Qt4, when we used
Q_WS_ defines instead of Q_OS_ defines.
This worked well for cases of #ifdef Q_DEAD_CODE_FROM_QT4, but less so
for cases of #ifndef Q_DEAD_CODE_FROM_QT4, where the code was actually
unconditionally included.
To make this even clearer, the defines have been replaced by checks for
1 or 0, with a comment describing how the code used to look in Qt4. The
use of constants in the check also makes it easier for editors to parse
the condition and show visually that the code is defined out.

Q_FOREACH over a QVarLengthArray or std containers
is a particularly bad idea (because of the deep

Use C++11 range-for (without qAsConst(), because
neither QVLA nor std containers are CoW).
Saves 5.7KiB in text size on optimized GCC 5.3

The algorithm used was:
- If possible, just declare the container const
- Otherwise, for first()/last(), use constFirst()/constLast()
and for front()/back(), to not destroy the use of the STL
API subset, use qAsConst()
Did some caching of function returns here and there, and
converted one 0 to nullptr as a drive-by.
Also saves almost 4KiB in text size on optimized GCC 4.9
Linux AMD64 builds.

Example on how to reproduce the bug with examples/widgets/mainwindows/mainwindow:
- Select the option "Groupped Dragging" from the "Main window" menu
- Drag and drop dock widget so there are at least three of them in
the same group of tabs (for example red, black, and green)
- Drag the tab bar of one of them outside to make it floating
(for example black)
- Drag away the group of two remaining tabs to make them floating
- Drack back the floating-alone (black) dock widget somewhere
back inside the main window.
=> The black dock widget is tabbed with the other instead of being
where we put it
That's because it was still inactive (skipped) in the tab group.

mistake for Q_OS_ ifdefs when quickly scanning the code. By renaming
the ifdefs we make it clear that the code in question is dead.
In incremental follow-ups, we can then selectively either remove, or
port, the pieces that are dead code.
