QtTest includes the whole module. Replace all such instances with

In Qt6 QNAM will no longer use bearer in any way so we deprecate it now.
Also mark bearermanagement-conditioned sections for removal in Qt6,
the _q_networksession property is part of how QNAM passes the
QNetworkSession around.

When network access is disabled, every QNAM request returns a
QDisabledNetworkReply instance, which emits error and finished
immediately. However isFinished() was still false, which could confuse
application code.

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.
Outdated header.LGPL removed (use header.LGPL21 instead)
Old header.LGPL3 renamed to header.LGPL3-COMM to match actual licensing
combination. New header.LGPL-COMM taken in the use file which were
using old header.LGPL3 (src/plugins/platforms/android/extract.cpp)
Added new header.LGPL3 containing Commercial + LGPLv3 + GPLv2 license
combination

In particular, set online state right upon construction of the
QNetworkAccessManager instance. Therefor, QNAM needs an instance of a
QNetworkConfigurationManager internally.
Before, this would only work properly if a network session was created.
Now, networkAccessible() returns the correct status.

As in the past, to avoid rewriting various autotests that contain
line-number information, an extra blank line has been inserted at the
end of the license text to ensure that this commit does not change the
total number of lines in the license header.
