Improve state handling in the Decoder
Use QMediaPlayer::PlaybackState to describe the state and trigger
correct state changes. Fixes a couple of test failures.
Also make our timing handling more consistent by switching all timings
in the Decoder over to using microseconds.
This fixes the last test failures in the qmediaplayerbackend
auto test.
