There's no need for having those in plugins and we can
further remove abstractions and simplify our code if
the code is directly compiled into Qt Multimedia.

Stop assuming that there are multiple plugins providing
the same service. We'll have exactly one plugin for a
service in the future and not provide several.
The reason is that we will focus on having one good and
functional multimedia backend per platform, not the mix
of many semi-functional ones as we have today.

From Qt 5.7 -> LGPL v2.1 isn't an option anymore, see
http://blog.qt.io/blog/2016/01/13/new-agreement-with-the-kde-free-qt-foundation/
Updated license headers to use new LGPL header instead of LGPL21 one
(in those files which will be under LGPL v3)

Qt copyrights are now in The Qt Company, so we could update the source
code headers accordingly. In the same go we should also fix the links to
point to qt.io.

Now that the MmRenderer implementation is ported to plain QNX, rename
the directories and files accordingly:
"blackberry" becomes "qnx", replace the Bb prefix with MmRenderer for
classes concerned with mm-renderer. The legacy alsa support for plain qnx
is now "qnx-audio".
