On Unix systems where the GUI event dispatcher uses a notification
system for socket notifiers that is out of band compared to select(),
it's possible for the QSocketNotifier to activate after the pipe has
been read from. When that happened, the ioctl(2) call with FIONREAD
might return 0 bytes available, which we interpreted to mean EOF.
Instead of doing that, always try to read at least one byte and examine
the returned byte count from read(2). If it returns 0, that's a real
EOF; if it returns -1 EWOULDBLOCK, we simply ignore the situation.
That's the case on OS X: the Cocoa event dispatcher uses CFSocket to get
notifications and those use kevent (and, apparently, an auxiliary
thread) instead of an in-thread select() or poll(). That means the event
loop would activate the QSocketNotifier even though there is nothing to
be read.

df757e30f816bee8bed5f65ff8e63cec57fe46b1 may have fixed this for one platform,
but different platforms appear to have different ideas on what constitutes a FD.
Just use the stream operator to avoid having to face this nightmare all the
time.

All read calls are now pulled from an intermediate buffer which is
populated from the asynchronous callback (this was a TODO previously, and
was breaking downloads of large requests). As a side-benefit, the use of
only async callbacks ensures fewer first-chance exceptions appear in the
debug output.

Added basic functionality to socket for WinRT. Even though not
all auto tests pass yet, this patch can be seen as a foundation
for upcoming work in this area. Reading from and writing to TCP
socket works and one can listen for tcp connections.

In a slot connected to readyRead, if the app detects that the
buffer size is too small and increases it, it expects that
readyRead() will be emitted again.
setReadBufferSize() doesn't re-enable the socket notifier when
calling from within readyRead, and readyRead itself was missing
the code to do it.

While QHostInfo and QNetworkInterface are implemented,
socket implementation is just a stub for now. Having
stub implementation is preferable over not having them
at all is because most applications will not build, if
sockets are not available. Even though they do not
do anything useful yet, applications can be compiled
and run to get an idea how network will work on WinRT.

The one in tst_QLocalSocket::writeToClientAndDisconnect just needed
proper ordering: that's what waitForDisconnect is for. At the same time,
we need to make sure we get the same message from all three
implementations of QLocalSocket::waitForDisconnect (and without the
useless space at the end).

The rule for a new override is that it must still work if the old
implementation is called. The catch is that any class that derives from
QProcess and isn't recompiled will still have QIODevice::open in its
virtual table. That is equivalent to overriding open() and calling
QIODevice::open() (like the tests).
In Qt 5.0, QProcess::start() called QIODevice::open directly, not the
virtual open(), so there's no expectation that a user-overridden open()
be called. With that in mind, simply fix QProcess::start to not call the
virtual open at all.
Similarly with QLocalSocket, the calls to open were always non-virtual.

QHttpSocketEngine fails to authenticate to an HTTP proxy that is using
Digest authentication and the CONNECT method (i.e. when you are
tunneling TLS over HTTP). The bug is due to a bad parameter being
passed to QAuthenticatorPrivate::calculateResponse - the requestMethod
parameter is passed in as "CONNECT " instead of "CONNECT" (note the
trailing space).
Because an MD5 hash is derived from this method when using the
qop="auth" flavor of Digest auth, the hash does not match the expected
value and authentication always fails in this configuration.

There is no 'name' variable, but apparently 'd->serverName' is correct
variable here. Some other methods take 'name' as an argument and have
similar code block, so I think this is a copy/paste error introduced
somewhere in the past.
